Significant Milestone Certificate

The Town of Essex would like to recognize residents who are celebrating significant milestone anniversaries and birthdays with a congratulatory announcement at Town Council and an official certificate. Please complete the following form to request a certificate for one of the following milestones:

25, 50, 60, or 75 year wedding anniversary
75, 80, 85, 90, 95, or 100 year birthday

An official Town of Essex Certificate will be signed by the mayor and delivered to the recipient by the date requested.
